Iacofano Library Scan Requests - 23 April 2026

Status

Request packet prepared on 23 April 2026. The Maria Iacofano Plain Dealer scan from the CPL Day 1 request was received and ingested as source-obituary-plain-dealer-maria-iacofano-1976. The Lawrence J. Iacofano Plain Dealer obituary and the 1932 Iaciofano John CPL necrology/cemetery record remain open Day 1 targets.

Manual-access continuation on 23 April 2026 re-opened the CPL request form in Safari and confirmed that the prepared Day 1 request text is still ready to submit as written. The same form still requires requester first name, last name, email, email confirmation, deceased/subject, date of death, additional information, and Google reCAPTCHA fields, so no automated or agent-submitted CPL request was made from this pass. A later browser-state check showed a CPL "We have received your inquiry" confirmation page in Safari, but the visible page did not expose the submitted subject or request body; keep Lawrence J. Iacofano and 1932 Iaciofano John open unless a CPL response confirms those items were included.

The Day 2 CPL packet remains queued behind Day 1, and the WEPL-first / Morley-fallback Lawrence request text remains unchanged. No WEPL live chat, phone, or email contact was initiated from this pass because that step still requires requester identity/contact details and live representational communication.

This file tracks the actionable free/public/library-first requests. Existing online index records remain secondary evidence after original scans arrive.

Repository Guardrails

  • Do not edit or overwrite existing files in 05_raw/.
  • Preserve every received scan as a new raw artifact.
  • One received artifact equals one source record.
  • New source and event records must use status: proposed.
  • Do not update 01_people/ unless the user explicitly accepts source records.
  • Do not create event records from index-only artifacts.
  • Create death events only from original scans that explicitly state a death date or death year and have sufficiently clear identity.
  • Preserve as-recorded names and spellings, including Maria, Iacofano, Iaciofano, Yaciofano, Biagio, and Blaise.

Contact Paths

CPL's form requires requester name, email, deceased/subject, date of death, an additional-information field, and CAPTCHA, so no automated submission was made from this pass.

Request text:

I am requesting an obituary/death notice copy from the News-Herald for this index entry:

Name: Iacofano, Lawrence J.
Age: 69
Publication dates: 23 August 1997 and 24 August 1997
Index source: News-Herald Obituary Index, 1990s Hol-I
Index URL: https://www.usgenwebsites.org/OHLake/newspaper/nhobits/199hol.html

Please provide an electronic copy if available. If both publication dates contain distinct, corrected, or service-arrangement notices, please include both.

Intake Workflow When Scans Arrive

  1. Check 02_sources/INDEX.md for duplicate titles, record IDs, and filenames before creating any source record.
  2. Preserve each received scan under the target 05_raw/pdfs/ filename below. If the target filename already exists, do not overwrite it. Choose a new unique filename and record the actual filename in document_details.
  3. Create one status: proposed source record per artifact.
  4. Use type: obituary for newspaper obituary/death notice scans and type: death_record for the CPL cemetery/necrology scan.
  5. Use input_mode: scanned_document_image unless the artifact has reliable embedded text.
  6. Omit citation: unless the library-supplied artifact includes a formal citation page.
  7. Put library citation details in document_details, including repository, index record ID/URL, publication, page/section, request source, and received filename.
  8. Add raw_text_file only if OCR or a manual transcription is created as a new 05_raw/text/{slug}.txt file.
  9. Cross-reference the existing online index source in notes; optionally update the online index source notes to point to the original scan source.
  10. Update relevant 07_leads/ records and this review bundle with the new proposed-source links, unresolved questions, and conflicts.
  11. Do not update 01_people/ until the user explicitly accepts the proposed source records.

If a library returns separate PDFs for the two Lawrence News-Herald publication dates, create separate source records for the separate artifacts, such as source-obituary-news-herald-lawrence-j-iacofano-1997-08-23 and source-obituary-news-herald-lawrence-j-iacofano-1997-08-24. If the library returns one combined PDF, use the single combined source target below and record both issue dates in document_details.

Reserved Ingestion Targets

Maria Plain Dealer, 1976

  • Raw file: 05_raw/pdfs/plain-dealer-obituary-maria-iacofano-1976.pdf
  • Source record: 02_sources/source-obituary-plain-dealer-maria-iacofano-1976.md
  • Status: received and ingested on 23 April 2026.
  • Type/input: type: obituary, input_mode: scanned_document_image
  • Existing index source: source-online-index-cpl-news-index-maria-iacofano-1976
  • Linkage:
    • Link person-mary-j-laurenzo-iacofano-1885 only if the scan confirms identity through children, spouse, address, maiden/family details, or other corroboration.
    • Otherwise use linked_people: [] and link relevant Mary/John leads.
  • Event rule: create a proposed death event only if the obituary states a death date or year and identity is strong enough.
  • Expected value: highest-priority test of whether Maria Iacofano is the canonical Mary J. Laurenzo/Iacofano.

Lawrence Plain Dealer, 1997

John Cemetery Record, 1932

  • Raw file: 05_raw/pdfs/cpl-cemetery-record-iaciofano-john-1932.pdf
  • Source record: 02_sources/source-death-record-cpl-cemetery-iaciofano-john-1932.md
  • Type/input: type: death_record, input_mode: scanned_document_image
  • Existing index source: source-online-index-cpl-necrology-iaciofano-john-1932
  • Linkage:
    • Link to lead-john-iacofano-1874 only if the scan ties him to Mary/Maria, Arthur, Random Rd/2025, or matching household context.
    • If the 1887 birth clue remains unsupported by household context, create or use a separate open lead such as lead-john-iaciofano-1887.
  • Event rule: create a death event only if a death date or year is explicitly stated. Do not create a burial-only event under the current event vocabulary.
  • Expected value: tests whether the 1932 Iaciofano, John hit belongs to Arthur's father or a separate 1887-born John.

Biagio Plain Dealer, 1994

Joseph Plain Dealer, 1991

Albert Plain Dealer, 1979

  • Raw file: 05_raw/pdfs/plain-dealer-obituary-albert-iacofano-1979.pdf
  • Source record: 02_sources/source-obituary-plain-dealer-albert-iacofano-1979.md
  • Type/input: type: obituary, input_mode: scanned_document_image
  • Existing index source: source-online-index-cpl-news-index-albert-iacofano-1979
  • Linkage: linked_leads: [lead-albert-yaciofano-1910] unless the scan proves a separate person.
  • Event rule: allowed only for an explicit death date or year.
  • Expected value: collateral-corroboration target for parents, siblings, spouse, residence, and possible Italian origin clues.

Lawrence News-Herald, 1997

  • Raw file: 05_raw/pdfs/news-herald-obituary-lawrence-j-iacofano-1997.pdf
  • Source record: 02_sources/source-obituary-news-herald-lawrence-j-iacofano-1997.md
  • Type/input: type: obituary, input_mode: scanned_document_image
  • Existing index source: source-online-index-news-herald-lawrence-j-iacofano-1997
  • Linkage: linked_leads: [lead-lawrence-iacofano-1928]; keep distinct from the Plain Dealer scan.
  • Event rule: add as support to the same proposed Lawrence death event only if it states the same death date or year.
  • Expected value: independent local-newspaper corroboration for Lawrence's identity, family relationships, death date, residence, and services.

Conflict Rules

  • Preserve document facts in extracted_facts even when they conflict with an index or prior interpretation.
  • Put index readings, older hypotheses, and alternate readings in transcription_notes or notes, not in as-recorded fields.
  • If Maria's obituary supports the canonical Mary identity, create a proposed source and possible proposed death event, then wait for user acceptance before adding death data to person-mary-j-laurenzo-iacofano-1885.
  • If Maria's obituary conflicts with current Mary facts, keep the document fact in extracted_facts, record the discrepancy in transcription_notes, and add a dispute note for user review.
  • If John's scan confirms 1887-1932 without tying him to Mary/Arthur, keep lead-john-iacofano-1874 open and isolate the cemetery John as a separate lead.
  • Do not merge the Lawrence/Palma/Blaise cluster into Arthur's Mary/John household without corroborating source text.
  • If a scan gives only publication date or service details, do not create a death event; keep those facts in the source record.

Acceptance Criteria

  • All received scans are preserved as new raw artifacts; no existing 05_raw/ file is edited or overwritten.
  • Each original scan has exactly one proposed source record, except when a library returns separate files for separate issue dates, in which case each file gets its own proposed source record.
  • Existing index sources remain secondary evidence.
  • No event records are created from index-only artifacts.
  • Proposed death events exist only where an original scan explicitly states a death date or year and identity is sufficiently clear.
  • 01_people/ remains unchanged unless the user separately accepts the proposed source records.
  • Relevant leads and iacofano-paternal-line-review-2026-04-23 document new evidence, conflicts, and next questions.
  • Indexes and validation pass after mutations.

Validation Commands

Run after any ingestion mutations:

python3 scripts/build_indexes.py
python3 scripts/validate_records.py

Optional machine-readable check:

python3 scripts/validate_records.py --json

Existing unrelated validation warnings may remain, but no new errors or new Iacofano-related warnings should be introduced.
